I have looked for a proper formatted contract for a couple days and I finally came across this company. I was not able to figure out how to properly use the site but I called and left a message and a lady called me back and took the time out to help me fill the form out step by step. Offers service for $50, has u pay. Then explains they are going to do half the described paperwork unless you pay another $100 and another $59 if you want the certificate.
