Your organization
Your organization, like many others, does not appear to be setup to provide assistance to the medically disabled. My wife needs a Power of Attorney, but the detailed requirements for this are not compatible with her medical condition. She suffered a debilitating Stroke last year that left her confined to a bed, with a difficult speech impediment, and a paralyzed right hand - rendering her unable to sign her signature. Since your standardized Power of Attorney form requires a notarized signature, the Notary was unable to verify not only here signature, but her medically qualified cognitive understanding of each detail of your printed form - evidently, Notaries are now required to also render qualified medical opinions concerning the comprehension of the content of applications, like your own. This requirement might be easily accommodated by someone "normal", but for someone disabled, it becomes an impossible barrier. You need to explain this formality to anyone expecting that merely downloading your application form - getting it routinely notarized - successfully completes the task of obtaining a Power of Attorney. This is not intended as a Review of your service, but a suggestion for improvement.
